When the Document Name or type or Linked Process name has been updated, this change may not be reflected in the edit screen for the Process.
Resolution
Either
Publish the Process
Or
Detach the Document or linked Process from the Process and reattach.

Detach the Document or linked Process from the Process and reattach.
