Issue
When trying to approve or reject a document or item from the drop-down menu in a library or list, the workflow does not respond.
Resolution
Use the "My Workflow Tasks" webpart.
Additional Information
When Content Approval is enabled in a library or list, the "Approve/Reject" option is added by SharePoint. It has no bearing on the approval status of documents in the workflow.
