Thank you for the direction. Unfortunately it does not appear to be throwing an error just simply not loading the data. It is almost as if the form never initiates the get list item option.

If that's the case, have you tested on how's it behaving when you load the view separately?
Again it might be the issue with the following -:
"There were some disconnects involving the SmartObject Item Reference between Form -> Workflow -> Form (Task State)."
Research the following:-
-Added a View 'Create' method to the submit button so that data would get saved.
- Created a SmartObject Item Reference in the Start Rule of the Workflow and made sure the Client Event was using said Reference and passing it to the Form
- Changed the Start Workflow rule to start the right workflow
