Check to see if there are any duplicate entries in K2 Claim tables.

[K2].[Identity].[ClaimTypeMap]
[K2].[Identity].[ClaimTypeMapping]

If you find duplicates in your Identity claims table as Nik mentioned, the best place to remove them would be:
K2 Management > Authentication > Claims > Realms
