Office 365 Nintex Forms Missing After Removing and Reinstalling App

Issue

After removing and reinstalling the Nintex for Office 365 Forms app, all custom forms are missing.

Resolution

1. Open the Nintex Forms app on the list where the custom form was previously designed and re-publish the form.
2. The custom form will now be restored to that list within SharePoint Online.
