Symptoms
Unable to see workflow "Folio", "Expected Duration" and "Priority" fields during "Start a workflow" form rule on certain machines.
Diagnoses
Developers only uses Chrome for design time. Found that the users who are not able to see it are on a older version of Chrome.
Resolution
Updating Chrome to the latest version (39.0.2171.65) resolves the issue.
