Symptoms
After upgrading the system to 4.6.9, we see a lot of issues. One of them being that the OOTB toolbar button icon doesn’t show up.
Diagnoses
The issue was caused by the old theme and new theme do not match.
Resolution
Correcting the new theme to match the old one resolve the issue.
