I have two parameters to pass query string values to these parameters?
How I can send values to these smartform parameters.
Diagnoses
I believe, SmartForms Viewer Web Part only accept static values currently. You should be able to set that values on the Web Part view SmartForms Connection Settings on the Web Part itself.

Resolution
You can use the following KB (http://help.k2.com/kb001351) to send query string parameter to SmartForm in SharePoint if you would like to pass during runtime.
