Hi Eric,

Sorry for the slow posting. I tested a couple of weeks ago and had no trouble with the system. The Pixel Perfect form is what I prefer for even though it took a little longer to create than the Responsive form. Pixel Perfect took about 15 minutes and the responsive took about 10. I guess the reason I like Pixel Perfect better is that it is easier to make the form look the way you want and the responsive for has you a little boxed in. I find both pretty easy to use though.

Having had some more playing I have the following feedback to add:  the aesthetics of the form (with rounded controls etc) differs from standard SharePoint. It would have been useful to have pulled in the standard css such as Office-UI https://blogs.office.com/en-us/2015/08/31/introducing-office-ui-fabric-your-key-to-designing-add-ins-for-office/
