Occasionaly my workflow will not start when a user submits a new request.

How is the done setting done, is it conditional or normal start. What version are you using.

Yeah more information needs to be provided. What are you doing in your work flow? Is it a list or site workflow? Is it scheduled?

It's a list workflow that starts when items are created.  Nintex 2013. Version 2.7.0.0
