Try removing the action and readding it. Sometimes when importing it's still trying to reference the old list you exported it from and it can't find it.

Just some friendly advice here, but I would avoid using special characters in your field names.   Even if they aren't considered illegal, I've seen many ill and often unexpected side effects.   Also try to keep your field names very short and concise as there are some interesting things (not good) that can happen when they become too long.

‌ Thank you. That worked!. ‌ Thank you. I did that when I created the form but and then changed the label afterward, I guess the workflow picks up the final label so I will have to be mindful of what I want my end product to be in the future.
