Nintex shared task form retrieval

I have a simple workflow for a Purchase Request.  The user fills out the form and submits the request.  In the workflow, there's a Request Approval action, and it formerly "shared" the form for the request, so it would display Approve/Reject radio buttons and below that, a view of the form showing the item details from the submitted form.  This was broken, though, and the Request Approval action is now linked to a custom form.  How do I get the original one back?  I need it to display the item details.
