Nintex 2013 Classic Form ListView column header

ok. So I created a list view and have the list filtered by a value in the parent section. Everything works fine. The only problem is the listview does not have the column headers. How do I get those to display?

Hi,
I have tested this functionality based on your screen shots and I am unable to replicate this issue. The columns appear for me.
Are you using a List or Document library or something else?
Not sure if an export of the form will help as this may be related to your environment. Are you using the latest build of Nintex Forms?
Is any custom JavaScript being used on the form?

Sounds like I should create a support ticket for this and not muck up the blog then.
