New Document Upload shows Blank New Form

We are using K2 Appit with SharePoint.  When uploading a new document the New Document page comes back blank.  The fix I found is in relation to K2 Cloud.  Is this issue also occurring in Appit?

    Although it is referred to as Appit, it is same as K2 Cloud. Appit is using the same code base as K2 Cloud. Please log a ticket with K2 Support to apply a fix on your environment.
