I am on the latest hotfix.

I am building up some simple InfoPath processes.  My task action dropdown in the IP form was working great but all of the sudden it is showing the actions duplicate.  So if I have actions "Approve" and "Deny" my dropdown will have:

""
"Approve"
"Deny"
""
"Approve"
"Deny"

Anyone run into this?  This is occuring on every activity in the process.  I inspected the dropdown in the form and everything seems fine.
