Hi Eric,
there will be a disruption in SharePoint Service as the update will perform some IIS Resets. However all workflows will continue running once SharePoint is back up. No workflows are cancelled or run into an error just because of an update of Nintex Workflows.
Just always keep in mind to also perform a database update if needed - as described in the installation instructions - as a new database schema after an update might affect certain workflow actions or functionality.
