Diagnoses
Only happen in IE9 with smartforms 4.6.11
Resolution
Workaroung by doing the following.
In IE 9 settings:
1. Unchecked the check box called ‘Display Intranet sites in Compatibility View’ on ‘Compatibility View Settings’ in ie9.
2. Close the browser and re-open again.
