How to Change a Workflow Task Form column type

I'm creating an approval workflow where there is supposed to be a, "Not Applicable" radio button in addition to the Approved and Rejected choices.

When I look in the Forms Designer for that Task Form, I see that the choice control for Status is connected to Decision field.

How do I add a "Not Applicable" radio button to that choice?

@MrBabakS ....it depends on what type of action you're using. In flexi task action, there is an option to add additional outcomes.

In the "Assign Flexi task" configure-action window, I click on "Edit Task Form" and I choose "Edit with Nintex Forms."
Then I choose the classic form.
Even though I had added an outcome "Not Applicable," the form still shows only Approve and Reject. The Add Additional Outcome and the Task Form don't seem to be connected.

The solution above worked; but it seemed like it hadn't because the Choice control was not expanded large enough to show the Not Applicable choice when in design mode.
