Error on adding an attachment

Sorry to bother you but I can't seem to figure out this error I'm having. It is directly related to adding an attachment to an item. It happens in my Edit Issue Form and New Issue Form (a helpdesk app), so I think it may be my attachment view. Can't find it.
Resolution
Issue: Attachments (working) but throwing an error when added.
Resolution: Duplicate Attachment View save method calls. The first add was completing successfully, and the second was throwing a null reference due to the data already being processed.
