Symptoms
Our problem is that when documents are uploaded via the Sharepoint 2013 app smo, they are sometimes uploaded as the correct impersonated user, and sometimes as "SharePoint app".
Diagnoses
We have not pinpointed the exact reason for this issue.
Resolution
Upgrading to 4.6.9 and reconfiguring the custom service broker fixed the issue.
