Create and Update Item in Same Document Library

Hi all,

I want to create folder inside document library. But I still have concerns about create item and update item in same workflow. 

This is my document library

I will create folder based on document set metadata, especially name = document number package number

I have developed nintex workflow like this

Update item configuration:

Create Item: 

 :

But folder still not created, 

Do you have any suggestions how to create folder after we update the item?
