I have a form for a list in SharePoint and I need to be able to collect data for each document and also send the documents with the data to a document library. Is there a way to do this currently?

Basically this is what I am asking:

Is there a way to have a document library form in the middle of a sharepoint list form? So that they get attached to the list item and uploaded into the document library and also I capture data for each document.
