I have a workflow in sharepoint online, it is very basic. I have an assign task action which works when I don't use the form. If I go into edit form on the action and create a form that all works fine however when the user submits a response I get the runtime error below, i don't get this error if I delete the form.

The workflow is for tracking compliance with some training, when someone is added into the SPO list the workflow will send them an email telling them to do some training, once they have done it there is a link in the email that they use to confirm they've done it which is the assign task action. I would like to use a form so it looks cleaner however I get that error whenever I use it.
