I would like to create a template.docx which included tick boxes, but when I use the document generator it converts it to a flat file pdf, is there anyway to still utilise the functionality?

The Document Generation action allow you to specify 3 output type for the generated documents.
If you want to generate Word Docs, your source must be Word docs and you need to select "Original file type".
