How to create a dynamic checkbox with a PDF template
Instructions
In Adobe Acrobat-
1. Add the checkbox on the PDF
2. Right-click on the checkbox and select 'Properties'
3. Select 'Options' tab and verify the Export Value to be "true"
In PDF Escape-
1. Add form field on to the PDF
2. Select 'Checkbox' from the dropdown
3. Right-click and select 'Object Properties'
4. Change 'Export Value' from "Yes" to "True"
