PDF could not be saved. Object Reference error

Executing SaveAsPDF on the form throws the following error: PDF could not be saved. Object reference not set to an instance of an object.
Symptoms
Unable to refresh the out-of-the-box PDF Converter Service Type and Service Instance from the SmartObject Service Tester. It throws the same "Object Reference" exception.
Troubleshooting Steps
Refresh both the PDF Converter Service Type and Service Instance from Workspace:-
1. Refresh the PDF Converter Service Type:
a. Go to Management Console > [Server]:5555 > SmartObjects > Services.
b. Click 'Services' to load the Service Type List'
c. Select 'PDF Converter' then click 'Edit'
d. Click on 'Save'
2. Refresh the PDF Converter Service Instance:
a. Go to Management Console > [Server]:5555 > SmartObjects > Services > PDF Converter
b. Select the 'PDF Converter' service instance then click 'Edit'
c. Click on 'Next' then 'Save'
