WF in the task list is stopped

I have a principal list with a WF, this WF create a task in the task list, now, in the task list I have a WF that start when task or item was modified, specifically when this task is answered (Approved or reject), but some tasks show me an error and dont start with this item or task was modified. I dont know why show me "Aplicación de Sharepoint" in the iniciator.

It's not a good practice to set workflows for tasks lists.

In your case I recommend you, after the answered task, to use the action to run your "other workflow"
