12.0.1 PageBuilder UI looks odd (few days after the upgrade)

We upgraded to 12.0.1 in Jan-2019, after few weeks, we noticed the PageBuilder UI looks like below (nothing we can really understand and change). Anyone else had this problem and know the root cause?

Try going to Themes > Update Out of Date themes.  That usually clears up wacky looking pages after upgrading.

Did updating the themes help with this issue?

We had production deployment this morning. Updating themes didn’t help, ended up upgrading to 12.1.3.
