The docs suggest the labels for feedback can be edited from Salesforce:

If you’re using Skuid on Salesforce, it’s possible to customize the content of the Feedback form (including headings, labels, and picklists) using the Salesforce Custom Label feature in Skuid. Feedback form labels are titled “Global_Feedback_Flabel name].”
