You need to add another view that reprent comment and durring the initial form it was be hidden and when is time for manager to approve or reject it must appear. The attach link below will show you step by step on how to add a comment :

It's definetly not an existing K2 Smartactions feature but it is a common requested feature by the business. The K2 out of the box SmartActions only allows for the action (decision), not the comment. Not sure there is any formal documentation actually saying this. Normally software companies do not include documentation on what their products do not do.
Before K2 email smartactions came out I wrote a custom component to do email approvals and added a comments tag. So technically its possible but it would be a development effort (if you aren't using K2 Cloud) and require you to turn off the native out of the box K2 Smartforms feature. However, these days we typically just add a note to the email notification saying if you want to add a comment, open the form and add it directly to the comments textbox on the form. Normally this is good enough for the business.
