Both declined and approved have same actions .
Below are the actions
Update item - Report System
Wait for item update
Foreach - Report (Update item - Reports)
Change State

Finally I come to know issue with update items.
Previously it's working fine, from past 2 days it's not working.
If i disabled update items action, everything working fine.
Please let me know, if i missing anything here.

check the permissions for the person who is approving/rejecting has appropriate permissions to update into that sharepoint list. If it is not the permission issue then check the field types whether you are updating with correct values.
And why you are using for each action without query list action before it.? Are you trying to update multiple items, if so how you are reading multiple items?

Yes, I have full permission.
Issue got resolved, I have used Action Update Multi Items instead of Update Items.
It's working fine now.
