Nintex Menu not showing in Site Actions SharePoint 2010.

We recently updated Nintex Workflow 2010.  Its only affecting one web application which is really odd.  We have deactivated and reactivated the site collection feature and site features and still nothing is happening.  We also have deactivated and reactivated it via Central Admin and still no go.  See screenshot below.

We are seeing the features be removed from the site settings page which is the activity to be expected; however, its not showing in the site actions menu as it should in a different site collection and web app in the picture below.

Any clues?

Fernando,

Thanks for the tip.  I did look at that and attempted a lot of the comments in there as provided in the summary of the question.

 

We did find a solution.  For some reason, the installer was updating but not activating on that web app.  We went in via powershell and retracted then installed it again using -force and everything worked like a charm.
