It depends on what the overall process is trying to achieve, can you provide some more context?

It’s to complete an online form and submit it to an internal department for approval.

If the approval steps are in the process I would title it Action <Form Name> Request (or Application etc depending on the form purpose). If approval steps are not included within the process I would title the process Lodge or Submit <Form Name>.
