userProfileLookup suddenly can't access userProfile data!

Hi, One of my forms suddenly stopped working , userProfileLookup can't access userProfile data and all fields are blank!

I  added new Person and text columns. No luck. 

what could possibly cause this issue for userProfileLookup() or Nintex form?

 

userProfileLookup(EmployeeNameCtrl, "SPS-JobTitle")

userProfileLookup(EmployeeNameCtrl, "Office")

    Hi,

    Do other forms that use this runtime function work?
    Has there been any update to the form that is no longer working?
    Has there been any permission changes on the list/site/parent site?
    Does republishing the form resolve the issue?
