I have a review assign task within one of my workflows and for some reason every so often the task will not kick off and I was wondering if there is away to get notified when the task don't kick off?

Can you elaborate further? Perhaps share a screenshot of the flow?
How does the workflow arrive at the review assign task? What are the conditions or logic?
Use the "Log to history" action to help trace your workflow logic.
The main reasons why a flow doesn't reach a particular point in the workflow is
- error in logic
- referring to incorrect variable value
- using the correct variable however the value assigned is incorrect.
