Any suggestions to get into form designer? I am a site collection admin and we have purchased the app. I emailed support, just looking for suggestions in the meantime.

+9That can happen sometimes. Usually I just uninstall and reinstall the app.
There's a twist though.
- Uninstall it
- Install it from the store
- Go to Site contents
- Cancel the installation
- Go back to the store and reinstall it
That should do the trick.
