They dont show up on the form and I dont see any options to add them. Am I missing something or is this a feature we dont have yet?

All, I've added a Customer Voice entry for this capability, so hopefully it gets some traction:
This is for external Business Connectivity Services (BCS) data, not regular internal list lookups.

Thanks Brent Ellis for creating the User Voice feature request. It helps us prioritise the next feature.
Our workaround for this at the moment is SSIS. I hate having to rely on a sync like that and creating duplicate data, but it's the only way we can use Nintex in O365 to allow users to make selections of "on-prem" data from our data warehouse.
As a result we're having use PowerApps for some things instead of Nintex. Microsoft have nailed the SQL gateway for PowerApps and PowerApps lets us connect to whatever data source we want pretty much.
