Are you using the Picture control to display the image? I'm assuming that you want the PDF file to look like this.

When I test this, I run into a couple of issues, It looks to be a combination of web browser and file security that prevent the image from appearing in the preview . To get around this, i have to grant K2 service account and the K2 site web application pool account full controls to the image folder and its image. Perhaps, you can set up a shared folder on K2 server and give k2 service and app pool account enough rights to it, this does require moving the images to this shared folder.
