Error on click to save form - cannot insert duplicate key row

K2 5.6 - Issue

When i click on save (i just added message in the form) the form it giving me below error:- 

Error %d, State %d, Procedure %s, Line %d Cannot insert duplicate key row in object 'Form.ConditionItem_Design' with unique 
index 'UX_Form_ConditionItem_Design_ID'.

 

Same form i am able to save on Production but not on QA.

I deployed form from Prod to QA, it deployed successfully. but same error on click to save the form.

I checked in K2 table Form.ConditionItem_Design but there is only 1 row available as i checked out the form.

 

Please help me, if any one have idea about this error.

If possible, I would recommend applying the latest CU and FP to rule out it being a bug that has been resolved.

Unfortunately, I have not come across this issue before.  I would recommend opening a Support case for assistance.
