Check out my blog post on Drawloop components. Components are designed to perform outcomes like this. You would need to build 1 group with 3 components. The 3 components would contain each address and a filter to align it with the correct city from your picklist. The group tag would then go into your document where the address needs to appear. All the commas and breaks can be included in your component too.
