you will have to design a work flow to send an email and workflow should invoke on the every item change trigger , you can configure the recipient and content in the work low email event .

So i have five fields in my form that i am interested to get an email when they changes. I have email format for each field when it changes. I can put a condition on start workflow about the field but i have no way to differentiate email for each field.
