Symptoms

I have changed to a different state in my workflow and some of the data in the form is missing, the action dropdown is missing and the header info for user info is missing. Checked all rules from initial configuration that was working and it all looks correct. Went through and reconfigured the event in the workflow and all required fields are mapped.
 

Diagnoses

Extensive troubleshooting resulted in cascading errors (fix one and get another, etc). Remote Services was recommended to assist in design.
 

Resolution

Customer called support by phone and reached Khan Vu, who walked him through a very simple test form setup with a simple test workflow and it worked. Customer concludes his project form was corrupt and intends to start his project over. We did discuss Remote Services and that his company does appear to have credits.
