Unique values in the form--How to enforce one form per requestor?

Hi, how do I enforce unique values in the form? Customer wants only one form submitted per requestor, which is set to default to "the current user".

Thanks a million, it works like a charm! It returns the name with the SP string in front but I think I can fix that one.
