Symptoms
Two separate workflows were firing in response to the submission of a form.
Diagnoses
It appears to be a case in which upon the renaming of a workflow the old named workflow still lived with start conditions tied to the same form as the newly named workflow.
Resolution
By deleting all instances of the workflow under the old name (after ensuring that no dependencies would be orphaned) we correctly saw only our desired workflow starting in response to a form submission.
