This may be possible if you have both language files installed, but that would be more of a SharePoint rendering issue and not the form.

The language files for Nintex supports the designer, but the output is dictated by SharePoint. You could format the labels to be German and English then if a button is checked, flips between the two. This is a ton of work, but its doable.
