This happens only when any of your validation rules have a syntax error. Please check what validation rule you added after which the form started displayed blank, remove it or fix it and the error will go away.

Thanks for your reply, yes I detected the problem, it was a syntax error in some calculated values. I fixed it and the problem disappeared.
