How does one initiate a new workflow instance from a SharePoint form?

To start a workflow from a SharePoint form, first ensure that you have a list/ library that has been appified, if this list does not require any K2 Forms, you do not need to check the Forms checkbox on the artifacts creation page:
After selecting the Workflow option, you will notice that you will be automatically set to start the workflow when a certain event occurs, in this example the workflow will start when an item has been added to the list. This means that whenever an item is created on the SharePoint list it is utilizing the new SharePoint form to start the workflow.
