we have a smartform that can submit items without any problems when using the administrator account, but the submit will fail when using new user accounts. Where should the permissions be set for any additional users?

You will need to add this user/group that this user belong in and grant Process Level > Admin or Start rights, as per:
Although error is too general to know if it is an issue with Start rights, perhaps the Application event logs on when K2 server when reproducing this issue will give better details.
