Smart Action Issue

We are facing issue with smart action on production server when a user replies to a task email notification for smart action , action is working perfect ,task getting Approved or Reject and in the same time users gets 2 messages from k2 server .
1- Success Message that his task is completed successfully.
2- Error message.
The K2 server could not find the Worklist item 1021_23. This item may have been actioned by another user.
The full error from the K2 server is 'the worklist item 1021_23 is no longer available or you do not have rights to open it.'.
This issue didn't happen when we went live it suddenly started to happen 2 weeks ago.
We tried , decrypting connection string and deleted SouceCode.MessageBus.EWS.ExchangeWebServiceConnection string but still getting same error.
Resolution
Kindly note that the SmartActions accounts for both prod and dev cannot be the same.
It is recommended to have 1 email account dedicated for 1 environment to ensure that the K2 server responds to the dedicated K2 SmartActions e-mail.
