Selecting one choice only and hide/disable the rest

I have an IT Hardware Request form and I have several choices for the user to choose from.  Desktop, Laptop, Laptop Bundle, Monitor, etc.  I only want them to be able to choose one piece of equipment.  If they choose Desktop then I want the other choices to either hide or be disabled and so on.  If they uncheck Desktop then they can make another choice, but not until they uncheck the previous choice.

If you added a choice control from the form designer, you can set the display format to option buttons. If you created the column in the list settings as a choice field, change the Display Choices setting to radio buttons in the column settings. Either of these will display all of the options but only allow one selection.
